Dr. Lawrence C. Kalker, MS, DPM, PC is a 1989 graduate of the Pennsylvania College of Podiatric Medicine. He completed his residency training in Podiatric Orthopedics at The Foot and Ankle Institute of Pennsylvania College in 1990. He founded Kalker Podiatry in 1991 and been practicing since that time in Newtown, Bucks County, and at a second location in Northeast Philadelphia.

Dr. Kalker has lectured extensively at the Temple University School of Podiatric Medicine in Philadelphia. He is an active member of the Newtown community. For 10 years, he served as president of the Lower Bucks County Chapter of the American Diabetes Association. He is a current member and past president of the Rotary Club, and is a current member on the Board of Directors of the Bucks County Association for the Blind and Visually Impaired.

Diabetic Foot Ulcer

A diabetic foot ulcer is an open sore or wound typically found on the bottom of the foot. This condition occurs in approximately 15% of patients with diabetes.

Ulcers develop due to a combinations of factors such as a lack of feeling in the foot, poor circulation, foot deformities, irritation, trauma and duration of diabetes.

Symptoms may include drainage in the socks, redness, swelling and odor.  Pain is not a common symptom since many diabetics who develop foot ulcers have lost the ability to feel pain due to nerve damage caused by elevated blood glucose levels over a period of time.

Heel Pain/Plantar Fasciitis

Heel Pain is very common and can have various causes such as stress fractures, arthritis, nerve irritation, cysts, tendonitis and plantar fasciitis. Plantar fasciitis occurs when the tissue that connects the heel to the toes becomes inflamed. The inflammation causes pain in the heel and will worsen over time.

A person may be prone to developing this condition if they have flat feet, very high arches, or poor alignment of the feet. You are also more likely to develop plantar fasciitis if you are overweight or if you spend a great deal of time on your feet wearing shoes that offer little or no support.

Allergic Contact Dermatitis

Allergic Contact Dermatitis is an allergic condition occurring when your skin is contacted by an allergen that you are sensitive to. When this happens an allergic reaction occurs, which may be in the form of a rash, dry or scaly skin.

Common causes of allergic contact dermatitis to the foot are poison ivy, oak or sumac as well as certain dyes and chemicals that may be found in footwear. Flatfoot (Fallen Arches)

It is very unusual to find a person who walks with both feet in perfect alignment. Often we walk on the inside or outside of our feet, or with our toes or heels rotated inward. These kind of walking patterns can cause not only problems with your feet but also your entire body.   The alignment of your feet sets the foundation for the entire body's alignment. One of the most common effects of improper alignment is called flatfoot or fallen arches.

This condition is characterized by a lack of an arch in the inner foot. This can be a genetic condition or the results of improper body mechanics. A healthy foot will be structurally able to support  the weight of the body but a flat foot is often unable to give the body the same support. Flatfoot can cause extreme pressure in the ankles, knees and hips.

Onychomycosis

Onychomycosis is a fungal infection of either the toenails or fingernails.  This ailment causes the nails to thicken, discolor, disfigure and split. When first looking at onychomycosis, you may think it is just a cosmetic issue. But without treatment, the toenails can become abnormally thick forcing the toe to press against the shoes, which can cause pressure, irritation and pain. If this disease progresses without treatment, onychomycosis may interfere with standing, walking and exercising.

Risks factors for developing onychomycosis include family history, advancing age, poor health, trauma, showering in communal showers and wearing shoes that don't allow proper airflow on a regular basis.

Athlete's Foot

Athlete’s foot is a fungal infection of the skin that causes scaling, flaking and itching of the affected areas. It often begins between the fourth and fifth toe. If left untreated, the skin may become highly sensitive to the touch. If Athlete's foot progresses without treatment, the condition can result in white, peeling skin and a milky discharge. Athlete's foot, which can be highly contagious, is typically contracted in damp places such as public pools, locker rooms and showers.

It is important to treat Athlete’s foot at the first sign of infection as the body can become vulnerable to other bacterial infections. The infection can also be transmitted to other parts of the body through bedding and clothes. Patients with serious cases of athlete’s foot may develop a rash that covers the sole of the foot. Selecting Proper Running Shoes

One of the first things a runner should do is select proper, supportive running shoes. Not wearing proper footwear while running can possibly lead to a variety of foot problems and cause injury.

Feet are typically categorized into three types: low/flat arch, normal arch and high arch. For those with low arches, you should choose a supportive shoe that will give you stability and motion control. For feet with normal arches, you should look for shoes with equal amounts of stability and cushioning to help absorb shock. What is a Night Splint and when does a patient need one?

A night splint is a brace that is worn at night when you go to sleep and it attaches to the foot as well as the ankle and lower leg. While you are sleeping, this device will stretch the plantar fascia ligament. It should be used when a patient suffers from plantar fasciitis, heel spurs , Achilles tendonitis, or drop foot. Foot Blister Prevention Tips

Foot blisters are one of the common injuries to athletes. They are fluid-filled bumps that develops on the foot when the shoe or sock is rubbing against the skin and creates the irritation. Besides being painful, blisters can change how an athlete runs, which can possibly cause additional injuries.

Here are some helpful tips to help prevent foot blisters:

  • Shoes should fit correctly. Wear the same socks, insoles and orthotics for the best fit.  Afternoons and evenings are the best times for trying on new footwear since feet tend to swell during the day.
  • Remember to break in your new shoes.  Wear shoes around the house for 1-2 hours to identify and discomfort areas.
  • Synthetic blends socks are the best to wear. Change socks that become damp.
  • Apply lubricants before putting on socks to areas that tend to blister.
  • Minimize moisture by using drying agents such as drying foot powders.

What are Plantar Warts?

Plantar Warts are noncancerous growths that appear on the sole, heel, or ball of the foot. Many plantar warts are diagnosed on their appearance. They tend to be rough and spongy and many are gray, brown or yellow with dark pinpoints.

Plantar Warts can occur when the human papilloma virus (HPV) enters the body through the skin on bottom of feet, usually through cuts and breaks. This virus is often present on surfaces such as tile floors or public locker rooms, showers or swimming pools.

Some people are more prone to the virus that causes plantar warts than others. Risk factors include repeated HPV exposure. For example, walking barefoot in common public areas. Having a weakened immune system may make you more susceptible to plantar warts.

Individuals often feel a "lump" on the bottom of the foot when standing. If left untreated, plantar warts can grow and may spread into clusters called mosaic warts. In severe cases, this can cause a change in gait or posture that results in leg or back pain.

Foot Blister Prevention Tips

Foot blisters are one of the common injuries to athletes. They are fluid-filled bumps that develops on the foot when the shoe or sock is rubbing against the skin and creates the irritation. Besides being painful, blisters can change how an athlete runs, which can possibly cause additional injuries.

Here are some helpful tips to help prevent foot blisters:

  • Shoes should fit correctly. Wear the same socks, insoles and orthotics for the best fit.  Afternoons and evenings are the best times for trying on new footwear since feet tend to swell during the day.
  • Remember to break in your new shoes.  Wear shoes around the house for 1-2 hours to identify and discomfort areas.
  • Synthetic blends socks are the best to wear. Change socks that become damp.
  • Apply lubricants before putting on socks to areas that tend to blister.
  • Minimize moisture by using drying agents such as drying foot powders.

Athlete's Foot

Athlete’s foot is a fungal infection of the skin that causes scaling, flaking and itching of the affected areas. It often begins between the fourth and fifth toe. The Difference Between Sprains and Strains

Sometimes when you experience pain when working out, you may wonder what kind of injury you may have. Most often, you'll have a sprain or strain of your muscle. How do you know the difference and how would you treat them?

A sprain is a stretch and/or tear of a ligament. A ligament is the fibrous band of connective tissue that joins the ends of two bones together. Ligaments help to stabilize and support the body's joints.

A sprain is caused by direct or indirect trauma that dislocates a joint out of position or overstretches it. In more severe cases the supporting ligaments can rupture. Usually this kind of injury happens when a person falls and lands on an outstretched arm, the side of the foot or runs on an uneven surface. Haglund's Deformity

Haglund's Deformity, sometimes known as the "pump bump" because it is prevalent among women who wear pump-style shoes. It may occur in one or both feet. This ailment can be caused by a combination of genetic and external factors and can lead to discomfort and sometimes pain in the back of the heel.

Hallux Rigidus 

Hallux Rigidus is a condition affecting the joint at the base of the big toe. You will notice pain and stiffness in the big toe if you suffer from this ailment. What is Tarsal Tunnel Syndrome?

Tarsal Tunnel Syndrome is a painful foot condition in which the tibial nerve is compressed as it travels through the tarsal tunnel. This tunnel is in the inner leg behind the medial malleolous, the bump on the inside of the ankle.

Symptoms of this condition include pain and tingling in and around the ankles, swelling feet, electric shock sensations, and pain radiating up the leg and down into the arch, heel and toes. Other symptoms include hot and cold sensations in the feet, a burning sensation on the bottom of the foot that radiates upward and a feeling that the foot does not have enough padding.

Osteoarthritis

Posterior Tibial Dysfunction

Posterior tibial tendon dysfunction (PTTD) is the inflammation or overstretching of the posterior tibial tendon in the foot. The function of the posterior tibial tendon is to support the arch of the foot. Morton's Neuroma

A neuroma is a thickening of the nerve tissue that may develop in various parts of the body creating nerve damage. Morton's Neuroma is the most common neuroma in the foot. It occurs at the base of the third and fourth toes.

Athlete's Foot

Athlete’s foot is a fungal infection of the skin that causes scaling, flaking and itching of the affected areas. It often begins between the fourth and fifth toe. If left untreated, the skin may become highly sensitive to the touch. If Athlete's foot progresses without treatment, the condition can result in white, peeling skin and a milky discharge. Athlete's foot, which can be highly contagious, is typically contracted in damp places such as public pools, locker rooms and showers.

It is important to treat Athlete’s foot at the first sign of infection as the body can become vulnerable to other bacterial infections. The infection can also be transmitted to other parts of the body through bedding and clothes. Patients with serious cases of athlete’s foot may develop a rash that covers the sole of the foot. What are Plantar Warts?

Plantar Warts are noncancerous growths that appear on the sole, heel, or ball of the foot. Many plantar warts are diagnosed on their appearance. They tend to be rough and spongy and many are gray, brown or yellow with dark pinpoints.

Plantar Warts can occur when the human papilloma virus (HPV) enters the body through the skin on bottom of feet, usually through cuts and breaks. This virus is often present on surfaces such as tile floors or public locker rooms, showers or swimming pools.

Some people are more prone to the virus that causes plantar warts than others. Risk factors include repeated HPV exposure. For example, walking barefoot in common public areas. Having a weakened immune system may make you more susceptible to plantar warts.

Individuals often feel a "lump" on the bottom of the foot when standing. If left untreated, plantar warts can grow and may spread into clusters called mosaic warts. In severe cases, this can cause a change in gait or posture that results in leg or back pain.

Ingrown Toenails

Brachymetatarsia

This condition is characterized by a short metatarsal. The foot had five metatarsals and this condition results in one shortened toe. This causes of brachymetatarsia is either genetic or situational meaning that you are born with this condition or it can be induced by premature closure in the metatarsal growth plate due to trauma.

A night splint is a brace that is worn at night when you go to sleep and it attaches to the foot as well as the ankle and lower leg. Do's and Don'ts of Diabetic Foot Care

If you suffer from diabetes, you already know that your healthcare requires special attention. When it comes to diabetes and your feet, many patients lose some sensation and therefore cannot detect an injury to their feet. Diabetics may also suffer from poor circulation which can slow healing and increase infection. There are many things you should know about the do and don'ts of keeping your feet in good condition.

Important Do's

  • Exam your feet daily. Look for any changes or broken skin.
  • Keep your feet clean.  Wash them with soap and warm water every day and towel dry them completely.
  • Wear shoes that fit properly and that protect your feet. Make sure you wear socks or stockings with your shoes.  See a foot care specialist to be fitted for comfortable shoes.
  • Wear loose fitting clothing. No garters or girdles that might restrict circulation.
  • Apply moisturizer to dry and cracked feet.
  • Check your shoes and socks before you put them on.  Look for any rough spots in shoes and rough seams in socks.
  • Make sure you are at a healthy weight. Lose weight if you need to.

 

Important Don'ts

  • No smoking. Quit if you haven't already.
  • Don't cut your own corns and calluses; see your podiatrist.
  • Don't apply heat to your feet or walk on hot surfaces.
  • Wear socks on cold feet and don't walk around barefoot.
  • Don't cross your legs while sitting.

 

By following these guidelines and visiting your physician regularly, you can avoid serious diabetic foot problems and have healthier feet.

Medicare Therapeutic Shoe Bill

Diabetic patients are eligible, under Medicare, to receive one pair of therapeutic shoes and 3 pair of inserts if there is a medical need. To receive payment from Medicare, it must be shown the patient needs the shoes for protection of insensitive feet known as neuropathy, poor circulation or foot deformity.

The following steps need to be taken for Medicare to cover your shoes:

The physician treating your diabetes needs to complete a certificate of medical necessity for diabetic shoes and documentation should be noted in your medical files.

The shoe supplier must have the order before Medicare is billed.

The shoes and inserts need to be prescribed by a podiatrist or other qualified doctor.

If you have Medicare Advantage Plan (such as a HMO or PPO), be sure to follow the plan's instructions for approval and purchase.

Each calendar year, Medicare will cover one of the following:

One pair of depth-inlay shoes and three pairs of inserts or;

One pair of custom molded shoes including inserts as well as two additional inserts.

Separate inserts or shoe modifications instead of inserts may be covered in some cases.

Keep in mind that Medicare does not cover deluxe features such as custom style, color or material.

To qualify for diabetic shoes you need to be covered under Medicare Part B. Besides being diabetic, you need to have one or more of the following conditions:

  • Partial or complete foot amputation
  • Past foot ulcers
  • Calluses that could lead to ulcers
  • Foot deformity
  • Poor circulation

 

The cost of your diabetic shoes will depend on if you have Medicare Part B and where you purchase your shoes. After you meet your deductible, you may be responsible for 20% of  the approved amount. Under the Medicare Advantage Plan, you may pay little or nothing, depending on the plan.
